ACP-HCC Education

 We provide access to a wide range of educational resources that support the practice of Advance Care Planning and the understanding of Health Care Consent.  There are online courses, webinars and handouts, as well as some face to face events that have been tailored to specific audiences:
Seniors/Older Adults, Families and Informal Caregivers as well as Health Care Providers.  

There is also a specific series of materials aimed at graduate and post-graduate Medical Students.   Use the dropdown option in the Education Navigator below to select the role that best represents your interest. ACP-HCC Education Navigator

HPCO is hosting this webinar presentation by Judith Wahl, Dr. Nadia Incardona and Dr. Jeff Myers to talk about the findings of a recent LCO research study and to inform providers how to best use forms within your organization related to HCC, ACP and GoC.  The cost is $25 and you may register here.
