Vision:The Seniors Health Knowledge Network, as the preferred knowledge exchange resource, improves the health and wellbeing of older adults and their caregivers.
Mission:At the Seniors Health Knowledge Network we connect people, ideas and resources to improve research, education, practice and policy, fostering health and wellness and improving quality of life and carefor older adults.

We ensure our success through
Evidence: Our Network opens doors to knowledge. It provides access to key experiential and research evidence through: library and information services; web-based resource centres; membership in communities of practice; participation in knowledge dissemination series and other learning events. Network members collaborate to create new knowledge and translate knowledge into resources and services for care providers and decision-makers.
Fostering Organizational Change: Through adaptation, translation and dissemination of research and innovation we demonstrate its relevance and applicability to practice and policy for older adults, caregivers, policy-makers and researchers. They know how to access, appraise and apply this evidence to their roles.
Facilitation:The Network harnesses the knowledge and skills of knowledge brokers, information specialists, researchers, members of communities of practice, coupled with knowledge exchange platforms, tools and strategies to facilitate the collaborative creation of new knowledge in the health and improved quality of life and care of older adults.
Integrating Quality Improvement: The Network contributes to improving quality of life and care for older adults in all settings by sharing best practices, and ensuring they integrate quality improvement standards.
Partnership and Collaboration: The network works in concert with others to promote optimal health, and quality of life, of older adults by leveraging resources, expertise and services, strategically identifying opportunities and supporting the pursuit of common goals to improve care.
Membership, Roles and Positions
The Network is guided by the Executive Committee with input from a broader Stakeholder group. Under the direction of the Executive Committee, the SHKN Secretariat (Executive Director, and support for technology, administration, knowledge brokering and information services) provides support and carries out the work plan.
Members: This group includes, but is not limited to, older adults and their families, paid and unpaid care providers, administrators, policy makers, researchers and learners. Network members can participate in Communities of Practice, use Information Services, access tools and resources, and participate in information sharing activities.
Stakeholder Group: The stakeholder group consists of representatives from the full continuum of care, various associations, institutions, government and funding bodies with a mandate for health and care of older adults and engagement in activities compatible with SHKN’s Guiding Principles and Success Factors. Community of Practice leads/ representatives of Community of Practice sponsor organizations are also members of this group. Stakeholders play an advisory role to SHKN. The group meets in person annually and consults via teleconference on an ad hoc basis.
At the discretion of the Executive Committee, membership to the Stakeholder Group may be extended to organizations outside Ontario. The Executive will approve applicants for membership and will review the membership every year to identify gaps and representativeness of the health care community of older adults.
Executive Committee: The main function of the Executive Committee is to develop and support the Vision and Mission and strategic plan of the Seniors Health Knowledge Network. It will meet biweekly or at call of Chair to guide the work of the Secretariat and identify funding opportunities and strategic partnerships.
Members of the Executive represent organizations/networks/groups from the Stakeholder Group who actively contribute resources to meeting the Strategic Plan. The Executive Committee membership includes representatives from the following sectors and partners:
· Regional Geriatric Programs
· Members at Large
· Ontario Interdisciplinary Council for Aging & Health (affiliated with COU)
· Communities of Practice Centres for Learning, Research and Innovation
· Ontario Research Coalition of Centres/Institutes on Health and Aging Alzheimer’s Knowledge Exchange
· Transfer Payment Agency
· SHKN Secretariat (non-voting)
Resources and Budget
Funding will be garnered from in-kind support, grants, project-based funding, research funding, donations, fee for service agreements, and strategic partnerships.
Process for decision-making will be based on achieving consensus amongst the full Executive Committee. Quorum will be 50% of the Executive.
